Scholarship Application Form The scolarship award is a minimum of $500.00. Applicants must complete this form and return it along with all documents requested below. Packages must be postmarket no later than July 20th., 2008 | | Eligibility Criteria
• Awardees must have been accepted or enrolled as a full time undergraduate student at an accredited institution of higher education. Graduate students or students pursuing professional degrees and international students with student visas are not eligible.
• Must be of Latino origin.
• Must be in good academic standing with a cumulative GPA of 2.5 or more on a 4.0 point scale or
3.5 or more on a 5.0 point scale.
• Must volunteer 15 hours for the organization during the Colombian Independence Day Festival and other activities.
• Close consideration will be given to students based on academic achievements, evidence of financial need.
• Close consideration will be given to students that demonstrate leadership qualities and have made positive contributions to the Latino community as defined by a record of involvement and participation in community, civic and extra curricular activities. |

Scholarships are to assist with the cost of tuition and books only. No funds are to be utilized for housing or other expenses. The scholarship will be paid directly to the institution on behalf of the recipient. Funds must be used for the 2007-2008 academic year. The recipient is responsible for having the school notify CUPULA in writing verifying enrollment. If this notification is not received funds will not be paid and the scholarship is forfeited. a. Transcripts of grades.
b. An essay describing your future plans, academic honors, awards, extra curricular activities, and how you and the Latino community will benefit from your endeavors.
• The essay needs to be typed or printed with black ink and need not exceed more than 500 words or
one 8.5 by 11 page.
c. One letter of recommendation from instructors, counselors, department chairs or community leaders that is able to describe your abilities and/or involvement with the community.
d. Proof of income. Attach copies of tax returns, W2 forms. In addition, list all sources of financial assistance that you have received in order to pursue your course of study. Deadline: July 20th, 2008 |
